Information for Childcare Providers - Use this brochure to explain the scheme to your childcare provider.- Armed Forces Variation and Leavers Form - Use this form to change the amount of vouchers you receive or leave the scheme. You can now make changes or leave the scheme through your online account. If you wish to make a change to your voucher amount or leave the scheme, the request must be completed by the last working day of the month to take effect the following month.

- Armed Forces Salary Sacrifice Form - You can now sign up on line, find out more visit the How do I join? section of this website. If you wish to still join the scheme by paper form, simply complete this form and return it to Sodexo at the freepost address. The Terms and Conditions are subject to change.
- Armed Forces Credit Request Form - Use this form to apply for a refund on any unused childcare vouchers.
- Childcare Provider Agreement Form - Your childcare provider will need to complete this form to receive your childcare vouchers as payment.
- Armed Forces Family Account Creation Agreement - Use this form to create a family account, so more than one adult (each with parental responsibility for the child) can receive their vouchers in one place. This can now be done through your online account, simply log in to your account or call 0800 066 5075 for more information.
