Information for Carers - Use this brochure to explain the scheme to your carer- Civilian Variation and Leavers Form - Use this form to change the amount of vouchers you receive or leave the scheme. You can also make these changes within your online account. The cut off date for changes is 10th of each month, if you want the change to happen in that month then the Effective From date should be 15th or before.

- Civilian Credit Request Form - Use this form to apply for a refund on any unused childcare vouchers.
- Civilian Salary Sacrifice Form - You can now join the Civilian childcare voucher scheme online! Simply call our customer care team on 0800 085 2875 and ask them to send you an invitation - this will include a unique link to take you through the process. Alternatively you can complete this form and return it to Sodexo at the freepost address. The Terms and Conditions are subject to change.

- Civilian Family Account Creation Agreement - Use this form to create a family account, so more than one adult (each with parental responsibility for the child) can receive their vouchers in one place.
