Variation and Leavers. You can now make changes or leave the scheme through your online account. If you wish to make a change to your voucher amount or leave the scheme, the request must be completed by the last working day of the month to take effect the following month.
Credit Request form – Armed Forces 2020 – Use this form to apply for a refund on any unused childcare vouchers. Please note refunds are subject to approval from the DBS MilPers MilOps CPMT Mgr and details will need to be provided for any refunds over one month’s deduction.
MOD-Family-Account-Creation-Agreement-form – Use this form to create a family account, so more than one adult (each with parental responsibility for the child) can receive their vouchers in one place. This can now be done through your online account, simply log in to your account or call 0800 066 5075 for more information.
